Students not planning to attend the commencement ceremony should print out this form, complete it, and return it to the College Registrar's office, 12 Erie Hall.
If you cannot present this form in person, please mail it to the College Registrar, 12 Erie Hall, 4651 College Drive, Erie, PA 16563-0201. Please verify your home (permanent) address on eLion.
Diplomas will be mailed to the home addresses of graduates if there are no holds on their accounts. Diplomas will be mailed from University Park. This form is valid only if all degree requirements have been met.
